Who needs to file a tax return?

Who needs to file a tax return in Israel? Independent workers (atzmaim) only? Salaried workers? What if I have income from stocks or other investments in Israel or abroad? What if I have only very minimal income, or none at all? 

Does filing a return mean hiring an accountant, or are there other options?
