Remote Nurse for Blood Pressure Monitoring Program
Location: Remote with the State license, located in Israel
Type: Full Time, afternoon/evening flexible
Education: License Practical/Vocational Nurse or Registered Nurse
Position Summary: Under general supervision and according to established policies and procedures, provide remote Blood Pressure (BP) monitoring for all patients enrolled in the Blip Care program. Communicates significant blood pressure changes and patient concerns to appropriate physician. Communicate with each assigned patients at least once every month and more when blood pressure is in the alert range.
Minimum Qualifications:
- Possesses a Nursing License in MD (or Compact).
- Knowledgeable about the basics of chronic health conditions, especially hypertension.
- Proficient with technology
- Comfortable with navigating computer platforms
- Comfortable with telephone outreach and engagement
- Excellent communicator (written and verbal)
- Highly organized and efficient
- Detail-oriented and good at multi-tasking
- Team Player
- Exhibits a strong sense of ambition, self-motivation and self-discipline.
Essential Functions:
Clinical Responsibilities
Provide blood pressure monitoring for patients enrolled in program by reviewing patients BP log.
Engage regularly with all assigned patients to encourage consistency with the treatment plan and ensure they remain within the data range set by their provider. Recognize alerts/flags in the portal and addresses them accordingly to policy.
Routinely call patients to discuss their blood pressure readings and overall progress.
Call patients about changes to their medication regimen, as directed by physician.
Document all patient interactions in the Blip Care portal and EMR.
Schedule regular monthly check-ins with the patients.
Monitor and analyze patients' data. Knowledge or wiliness to learn RPM billing CPT codes (99453, 99454, 99457 and 99458)
Communication Responsibilities
Call each assigned patient every month to discuss their trend and provide education.
Call each patient who is in alert range the same day.
Communicate readings in alert range to appropriate physician.
Communicate patient questions or concerns to the appropriate physician.
Discharge patients when necessary and communicate information to appropriate physician.
Technical Responsibilities
Ensures all blood pressure monitors are always in working order.
Communicate with patient and/or vendor regarding troubleshooting of non-working devices.
Have thorough understanding of all error codes associated with the monitor.
Ensure blood pressure readings are being transmitting into the Blip Care portal in real-time.
Have thorough understanding of technical and professional billing codes.
Additional Responsibilities
Assist patients with insurance/billing related questions about their blood pressure monitor.
Attend weekly or bi-weekly meetings to discuss program updates.
Other duties as assigned by the management team.
