Position Summary
The Care Cycle Coordinator (CCC) Account Manager runs a Home Health region for Infinite Services. This leader is responsible for the high-quality, compliant and profitable delivery of therapy services to patients at home and in assisted living facilities (ALFs), managing every patient's care cycle from first referral to final payment.
Working with autonomy, the CCC Account Manager builds and leads the regional team, grows referral partnerships, and owns the region's budget, billing, systems and results, including patient census, revenue and gross and net profit. Central to the role is proving that Infinite's therapy programs deliver faster, higher-quality recoveries and fewer hospital readmissions, and using those outcomes to win and keep partners. The role suits a decisive, business-minded and creative leader who builds strong teams, holds them accountable, and serves as the key link between regional operations and department leadership.
What the Job Entails
Regional Ownership and Growth
- Own the region like a small business: patient census, revenue, costs, and gross and net profit.
- Develop and carry out a regional business plan, including yearly goals, budget and growth targets, with department leadership.
- Build clear processes so every patient moves smoothly from referral to intake, authorization, staffing, care and billing.
- Find creative ways to grow, through new referral channels, services and partnerships.
Team Leadership
- Lead, supervise and mentor the regional team, with one person responsible for each part of the care cycle:
- Business Development:
finds and grows referral sources and markets Infinite's services.
- Patients Coordinator (Intake):
takes in referrals, handles admissions, and is the main contact for patients and families.
- Providers Coordinator (Staffing): recruits, schedules and retains the therapists who care for patients.
- Partners Coordinator (Authorization): obtains insurance authorizations, checks eligibility, and manages payer and partner relationships.
- Billing: handles claims, invoicing and collections, and fixes billing errors.
- Decide which roles the region needs and when to add them, and hire the right people for each.
- Set clear goals for each team member, run weekly department meetings, and conduct performance reviews every four weeks.
- Balance caseloads and staffing to meet service demand, and retrain or replace team members when needed.
- Build a positive, accountable, high-performance culture that encourages ownership and continuous improvement.
Patient Care and Outcomes
- Make sure every patient has the right authorization and the right therapist in place before care starts.
- Educate patients and families about therapy services and what to expect.
- Hold regular patient check-ins to confirm services meet expectations, and adjust care plans as needed.
- Track recovery outcomes and readmission rates, and use the results to show the value of Infinite's care.
Provider Relationships
- Monitor therapist performance, utilization and documentation timeliness to ensure quality and compliance.
- Make sure therapists understand expectations, workflows, performance benchmarks, and Infinite's focus on measurable recovery outcomes.
- Partner with clinical leadership to resolve provider issues and improve patient care.
- Lead initiatives to improve provider satisfaction, engagement and retention.
Partner Engagement and Business Development
- Lead business development personally at first, then through the business development team member as the region grows.
- Build and maintain relationships with referral sources, including physician offices, ALFs and other healthcare partners.
- Identify the real decision-makers at each account (discharge planners, case managers, physicians and administrators) and learn what they need from a home health provider.
- Act as the regional liaison for partner satisfaction and service continuity, and resolve complaints quickly.
- Track every lost referral and its reason (insurance, staffing, timing or patient choice), and fix the causes.
- Represent Infinite professionally, reinforcing its reputation for quality and reliability.
Budget, Billing and Finance
- Create and manage the regional budget, and take part in forecasting and cost-control initiatives.
- Own regional revenue, gross and net profit, and provider-cost targets.
- Oversee billing for the region, ensuring accuracy and timely submission in line with payer and state regulations, so that care delivered is care paid for.
- Monitor key financial metrics, including billed hours, cost per provider hour, utilization and denials.
- Control costs such as staffing and overtime without affecting patient care.
- Work closely with internal departments, including Billing, Finance, PCD and Talent Acquisition.
Systems, Data and Reporting
- Own and develop the region's systems, reporting and dashboards.
- Review data daily and weekly, tracking referrals, admissions, census, billed hours, utilization, provider cost and documentation compliance.
- Analyze trends and variances, and lead data-driven improvements to efficiency.
- Ensure data integrity and compliance with payer, regulatory and patient privacy requirements.
- Present monthly financial and operational reports to leadership, with results, problems and corrective actions.

- 5+ years of experience in home health or a similar healthcare setting, including 3+ years in a supervisory role.
- Proven ability to lead teams, manage cross-functional workflows and drive measurable results.
