EMA Care is looking for a dynamic Nurse Case Manager for management of students and private clients (individuals and groups). This job is remote with flexible hours. Room for professional growth and a great opportunity to work with a professional and supportive team.

Duties, Responsibilities, & Skills

• Assesses clients regarding their chief complaint
• Must have excellent physical assessment skills
• Experience working with medical protocols

• Communicates care plan with client, EMA Care team and others as medically necessary
• Oversees case management for assigned clients
• Collaborates with client/client staff to ensure care plan is understood
• Provides clear direction for administration of care plans
• Follows up with client/client staff regarding current cases for schools
• Documents appropriately
• Experience working with Word, Excel, and Cloud based documents

Qualifications

• Registered Nurse, Nurse Practitioner, or Physician Assistant
• Licensed in Israel or equivalent Western country
• Native-level English
• Conversational Hebrew
• Relevant clinical experience of at least 2 years
• High-level interpersonal skills
